This form is to be completed for each Camper participating in Middle School Camp.
After you register, your parish group leader will give you additional information including logistics and fees.
Camp has limited capacity, both in total and for each parish. If your parish's spots fill up, please still register! You will be added to a waiting list. Your parish leader will reach out when additional spots open up.
